Human Development Services

Human Developmental Services provides services for individuals with mental retardation or developmental disabilities.  This division gives to those whom it serves, a foundation and basic building blocks to address their disabilities.

Key Services:

 

bulletADH (Adult Day Habilitation) - A variety of day programs designed to enhance the individuals capacity to function independently within the community.
bulletCase Management - The coordination of support services located within the community necessary to maintain the individuals existing service needs.
bulletFamily Home - The 24 hour, 365 days a year residential placement, which is provided by independent contractors in their private residence. This service is available to both SCL and DSS recipients.
bulletStaffed Residence - The 24 hour, 365 days a year residential placement, which is provided in Agency operated facilities with full time employees. Available to SCL and DSS recipients only.
bulletRespite - The temporary residential care of individuals to provide relief to families/consumers or to assist in special situations. Respite may be provided on an hourly or daily basis.
bulletSupported Employment - The assistance to individuals in locating and securing meaningful employment in the community at competitive wages. Also involves follow-up after placement to enhance success.
bulletBehavior Management - Very specialized services, which are provided to individuals to assist them in developing new or modifying existing behaviors which impede their personal development.
bulletPASARR - An assessment process entitled to nursing and personal care home residents, who may have developmental treatment needs which exceeds the capacity of their existing provider.
bulletKEIS/POE (First Steps) - The service is designed as a Point of Entry for the First Steps Program, in which children 0-36 months are identified, assessed and referred for further treatment.
bulletSupported Living - A grant program designed to provide ongoing and recurring services for applicants based on need. Decisions on awards are made by a supported living council, who are appointed by the Governor’s Office.
bulletIndividualized Support - Similar to Supported Living, but awards are reevaluated annually based on funds and can be for various expenses and activities needed to maintain the individual in the community.
bulletCommunity Living Supports - A program designed to provide additional support services to SCL recipients, who reside outside of a family home or staffed residence.
bulletCrisis Response - A restricted account designed specifically to respond for crisis and emergency situations, in which individuals are faced with institutionalization or without any other means of support. Services are short term and non-recurring.
bulletImpact Plus - A new Medicaid sponsored service designed to provide multiple services to individuals under the age of 21, who have traditionally been underserved by other sources.

 

Procedure for Referral:

Any Individual or organization may refer a person for any of these services by calling for an appointment.
